O. TRANSPORTATION RELEASE INFORMATION FORM Every student utilizing Agency transportation shall have this form on file. This form is delivered by the driver to the student’s parent or guardian and contains inf ormation regarding the conditions of the student’s safe release by the driver fully completed within 10 school days of the student being added to the route.
